Oddly the remains of only one victim of Roman Crucifixion, a Jew named Johanon, have been found. The remains indicate that he was nailed just as you describe.
Normally roman crucifixion was an excarnatory form of punishment with the bodies left on the crosses after death for the remains to decompose in plain sight and the bones to be scattered. this is meaningful in light of the contention in the Fourth Gospel (';John';) that Jesus was crucified before the start of Passover and that Pilate allowed the condemned to receive a coup de grace and the bodies to be buried prior to sunset (the start of Passover) in order to appease the sensibilities of the Jews with regard to not profaning the High Holy Days. you are correct in thinking it was 3. The only reason they gave you the nail in the feet was to prolong the torture. Crucifixion kills you by suffication; not pain or bleeding out.
